Nice, impressive and very well preserved!
Before us is an extraordinary item: a prayer book, Ashkenazic version with Kabbalistic Kavanot by the Ari! These Kavanot are unknown!
The Siddur contains Kabbalistic Messianic-influenced supplications and Piyyutim. It seems their writing was influenced by a Messianic group – maybe, the members of the group gathered around the Ramchal. Northern Italy, the 18th century.
The uniqueness of the version of the prayer book is the reason for it being handwritten rather than printed. The owner of the Siddur used to say a unique version of the prayer – a version that did not appear in the popular Ashkenazic prayer books – therefore, he wrote the Siddur by himself or ordered a scribe to do so. This is an exceptionally rare item! Experts to whom we have shown the Siddur have determined that complete handwritten Ashkenazic prayer books from the period of the one before us are extremely rare. They had never seen such a Siddur. In addition, such an item has never been seen in auctions!
We have yet to determine the identity of the owner of the Siddur; however, we have no doubt that it was a great Tsaddik and it is most likely that he was a well-known prominent rabbi!
The Siddur contains the prayers for the days of the week, Sabbath, Shalosh Regalim, Rosh Ha'Shana (including the blowing of the Shofar) and Yom Kippur, Hoshanot and Seder Ta'aniyot. It also contains rare supplications and Piyyutim and comments regarding the Kabbalistic intentions (Kavanot) of the prayers.
The prayers are written in square Italian script; the comments in partially-fluent Italian script.
Attached is the detailed opinion of Prof. Shlomo Tzuker who is an expert of Hebrew manuscripts.
There are also short, interesting comments on the prayers themselves, which were added between the lines in different script (for example, on leaves 107, 121, 122 and more). On the margins of the leaves of the "Al Chett" prayer of Yom Kippur, short comments explaining its content according to rare sources were added. For example, the sentence "Al Chett She'Chatanu Lefanecha Be'Ones U'Ve'Ratzon" is explained by the commentator as "gentiles had forced him to disobey one of the commandments of the Torah, and if not – they would kill him, he should disobey and not be killed except for Avodah Zarah, Giluy Arayot and Shfichut Damim". The source of this commentary is the Romania Machzor [Venice 1523].
At the end of the book, an "Index" (2 pp.) and then an additional leaf (by another writer?) with Parashat Ha'Akeda and the prayer in Italian script (2 pp. the last line of the prayer is missing).

175 [2] leaves.
Condition: Good. Impressively well-preserved. Ancient parchment binding. Thick, bright leaves. Stains of use. Leaf 7 is missing and there is an empty leaf instead of it. In the five first leaves and the last three leaves, an omission in the inner margins which has been professionally restored. A few leaves are worn.
